Who Can Use This Service

Is This Right for You?

Don't worry - the requirements are straightforward. If you're already working with a case manager and enrolled in Community First Choice, you likely qualify. Here's what you need:

You must qualify for Community First Choice (CFC) and have Remote Supports authorized in your person-centered support plan

Your case manager and support team agree that video-based support would help you meet your goals

You want to receive help through video calls and a tablet rather than having someone come to your home

Remote Supports is not covered by Colorado Medicaid in provider-owned or provider-controlled HCBS settings or group living/congregate arrangements

Case managers can refer anyone who meets these requirements - we'll help with the assessment and setup

We believe everyone deserves to live where they choose. Pathway Home Health was founded by healthcare professionals who saw too many people forced into facilities simply because they needed occasional support.

Technology has changed what's possible. Through video calls and smart devices, we can now provide virtual prompts, reminders, and coaching that help people stay safe, independent, and connected to their communities.

We primarily serve people enrolled in Colorado's Community First Choice program, helping members access Remote Supports as an alternative to in-person care.
